- Change of Address
- Tuition Appeals and Late Drops — Use this form for tuition appeals or to petition to drop a class after the tenth week of a semester.
- Re-Enrollment Form
- Certificate of Good Standing Form- please reach out to Nathan Blackburn at blackburnn@ohio.edu to request a PDF of this form. A signed copy of this form must accompany an application for non-degree status as a Graduate Transient or Graduate International Exchange student. The Graduate College will accept this form as documentation of good standing at the graduate level in lieu of university transcripts.
